Information for Patients: Patients should be advised that: Some people, particularly young children, may experience difficulties swallowing the tablets whole. "Without protein [in the body] being properly produced, the aorta stretches, but doesn't necessarily come back to normal size, " says Larsen. As the aorta gradually enlarges, patients with Marfan syndrome risk two life-threatening conditions: an aneurysm, which stretches and bulges the aorta wall and can eventually burst, or a dissection, where the aorta attached to the heart stretches and pulls the valve, causing it to leak. Physicians carefully monitor Marfan syndrome patients for aneurysm formation and often recommend aortic root replacement surgery, depending on the size of the aneurysm and how well the aortic valve is functioning. Historically, surgeons would replace the parts of the aorta with a graft that includes a mechanical valve. Dr. Nilto De Oliveira, a UW Health cardiothoracic surgeon, offered Larsen an alternative: a more advanced, aortic valvesparing procedure. This virus shows a considerable genetic heterogeneity among isolates from all over the world. Natural history and response to treatment is related to genotype of HCV in a particular patient. In this study we evaluated the genotypes of patients with HCV in this center, which is the largest referral center in southern of Iran. Methods: In all cases referred with HCV infection diagnosed with positive anti-HCV antibody by ELISA and Blot ; to this center Shiraz University of medical Sciences Liver Clinic ; during last two years, interviewed and asked about their risk factors for infection, and studied genotype. HCV RNA was extracted from their serums and HCV genotype was determined by RFLP method. HIV positive patients co-infection ; were excluded from the study. Results: Among 408 patients with exclusive HCV hepatitis, 336 ones 82.4% ; were male and 72 17.6% ; were female with mean age of 36 years range 7-69 ; . In our study the females were significantly younger than males The mean age in females were 3213 in comparison to 3711 in males, p 0.001 ; . 326 ones accepted blood sampling for genotype analysis which genotype of 120 individuals were untypable. In the remaining 206 HCV patients, 89 ones 43% ; had 3a strains, 85ones 41% ; 1a strains and 32 ones 15% ; had 1b strains. Distribution of patients' genotype according to different risk factors was accessed. Consuming the thalasemia as a risk factor, a significant difference between the patients with and without thalassemia DF 3, p 0.001 ; . This distribution in patients with tattoo, multiple sex partners and also hemophilia was not differ from those without p 0.13, p 0.81, p 0.097, respectively.

This study shows our results of surgical treatment of liver hydatid cysts during a 5-year period. Methods : A prospective study of 21 patients operated on in a 5-year period 1999-2003 ; , in Dubrava University Hospital in Zagreb, Croatia, with hepatic hydatid cyst. All patients were pre-operatively treated with albendazole. In 12 patients, total pericystectomy without opening the cyst cavity was performed, 9 open and 3 laparoscopic. In the other 9 patients, partial pericystectomy was done, 6 open and 3 laparoscopic. Results : There was no mortality after 5-65 months follow-up, but in 1 patient, in the open partial pericystectomy group, recurrence of the disease occurred after 3 years. When a laparoscopic procedure was done, there were no complications or recurrence. The median operative duration for open surgery was 100.0 min range 60.0-210.0 ; , and for laparoscopic surgery 67.5 min range 60.0-120.0 ; . The median length of hospitalisation for open surgery was 8.0 days range 7.014.0 ; , and for laparoscopic surgery 5.0 days range 4.0-7.0 ; . Conclusion : Total pericystectomy without opening the cyst cavity, preceded by pre-operative albendazole therapy is the method of choice for hepatic hydatid cyst treatment. Despite the small group of patients, our first results show laparoscopic total pericystectomy without opening the cyst cavity, in the treatment of hepatic hydatid cyst, as a good alternative to open surgery in selected patients.
