Introduction: IgA nephropathy IgAN ; is the most common primary glomerulonephritis with various pathological and clinical phenotypes in the world. However, the exact etiology of IgAN has not been clearly elucidated. It was suggested that the formation of deglycosylated IgA1-containing macromolecules might play an important role in the process of the disease. Substantial evidences also demonstrated that IgAN might be closely associated with aberrant mucosal immunity. Recent study suggested that secretory IgA SIgA ; might play a pathogenic role in the pathogenesis of IgAN. The aim of this study is to investigate the level and characteristic of serum SIgA in IgAN patients with different pathological phenotypes. Methods: With enzyme-linked immunosorbent assay, the levels of SIgA in serum, polyethylene glycol 6000 precipitated serum macromolecules and IgA1-binding proteins obtained from desialylated IgA1 DesIgA1 ; Sepharose and desialylated degalactosylated IgA1 DesDeGalIgA1 ; Sepharose columns were detected in 47 patients with IgAN and 48 normal controls. The association between the level of SIgA and the pathogenic phenotypes of IgAN was investigated. Results: The level of serum SIgA in patients with IgAN was significantly higher than that of normal controls 16.809.37 g ml vs 9.965.83 g ml, P 0.001 ; . The level of serum SIgA in patients with focal proliferative sclerosing IgAN fpsIgAN ; was much higher than that in patients with mild mesangial proliferative IgAN mIgAN ; 22.719.33 g ml vs 11.144.90 g ml, P 0.001 ; . The level of serum SIgA correlated with the level of serum creatinine R 0.509 ; , degree of proteinuria R 0.643 ; and creatinine clearance R -0.454 ; in patients with IgAN. Serum SIgA could be detected in serum macromolecules and the concentration of SIgA was higher in patients with fpsIgAN than that of patients with mIgAN and normal controls P 0.005 and P 0.001 respectively ; .
